  • Boosting the high-quality development of day surgery in the hospital driven by smart information big data platform

    In recent years, day surgery has developed rapidly in China. Day surgery management has shifted from extensive to refined, but there are still many problems in the service system of day surgery in Chinese hospitals. In order to further optimize the allocation of medical resources, improve the level of medical service capacity, and build a “patient-centered, safe, efficient, and orderly” day surgery service system, Northern Jiangsu People’s Hospital has integrated big data, mobile internet, and artificial intelligence since 2019, creating a smart information big data platform. This paper summarizes the experience of Northern Jiangsu People’s Hospital in promoting the high-quality development of day surgery services in the whole hospital from five aspects of top-level design, diagnostic and therapeutic process, medical quality and safety, medical supporting services, and supervision mechanism, with a view to providing reference for the implementation of overall management of day surgery in the hospital.

  • Action guideline on promoting the high-quality development of rehabilitation services in public hospitals

    According to a guideline on promoting the high-quality development of public hospitals issued by the General Office of the State Council and a guideline on accelerating the development of medical rehabilitation, a high-quality and efficient rehabilitation system should be established, and efforts should be made to promote the high-quality development of rehabilitation services in public hospitals. We propose an action guideline for promoting the high-quality development of rehabilitation services in public hospitals, which is based on a Delphi method questionnaire on the opinions of front-line clinical rehabilitation experts, and integrated with relevant literature and evidences. This guideline introduces 31 specific action plans from 7 dimensions, including standardization of basic rehabilitation settings, standardization of clinical rehabilitation techniques, improvement of the tertiary rehabilitation system, standardization of emergency rehabilitation, standardization of rehabilitation education, standardization of rehabilitation management, and promotion of scientific and technological innovation in rehabilitation medicine.

  • Construction of evaluation index system for day surgery quality and safety based on Delphi method and analytic hierarchy process

    Objective To establish an evaluation index system for the quality and safety of day surgery, and provide standard and professional quantitative bases for monitoring and management of day surgery quality. Methods From December 2021 to May 2022, based on the frame work of the three-dimensional “structure-process-outcome” quality model, two rounds of Delphi method interviews were conducted with 15 day surgery-related management experts, and analytic hierarchy process was used to calculate the weight of each index. Results The effective questionnaire recovery rates of the two rounds of surveys were both 100%; the expert authority coefficients were 0.94 and 0.98, respectively; the variation coefficients were 0.00-0.30 and 0.00-0.24, respectively; the Kendall coordination coefficients were 0.382 (P<0.05) and 0.266 (P<0.05), respectively; and the consistency ratios of all levels were less than 0.1. The final day surgery quality and safety evaluation index system included 3 first-level indicators, 10 second-level indicators, and 43 third-level indicators. Conclusions The quality and safety evaluation index system of day surgery is scientific, reasonable and comprehensive. It can provide a reference for the quality and safety evaluation of day surgery and solidly promote the high-quality development of public hospitals.

  • Research on the development of restricted medical technology management in medical institutions from the perspective of high-quality development

    Under the background of high-quality development of public hospitals and from the perspective of medical technology management, this paper discusses the high-quality development path of medical technology management in West China Hospital, Sichuan University. By sorting out the development context of restricted medical technology management in this medical institution, it concludes that there are four problems in restricted medical technology management at present: lack of standardized management system, non-unified surgery grading management, technology management information level to be improved, and the after-effect evaluation and transformation mechanism of medical technology being not perfect. It is proposed that establishing standardized management system, unifying surgical grading catalogue, constructing intelligent information system and establishing achievement transformation mechanism are important ways to strengthen medical technology management.

  • Practice and development of day surgery for municipal third-level public hospital under guidance of national hospital performance appraisal system

    ObjectiveTo analyze the efficacy of promoting the development of day surgery in a municipal third-level national hospital under the guidance of national hospital performance appraisal system. MethodThe annual data relevant day surgery in the Yibin Second People’s Hospital were collected from 2016 to 2022, which were divided into three stages: 2016–2018, 2019–2020, and 2021–2022. The trend and correlation of the performance appraisal indicators were analyzed. ResultsThe day surgery in the Yibin Second People’s Hospital started in 2018, and its proportion in the elective surgery was only 1.2% initially, then increased continuously after the implementation of performance appraisal system, and was up to 34.7% in 2022. From 2016 to 2022, the proportion of discharged patients underwent surgery in the entire hospital increased continuously from 22.4% in 2016 to 35.7% in 2022, and the average hospital stay in the entire hospital gradually decreased from 10.9 d to 8.1 d, which both had a significant linear correlation with the proportion of day surgery in the elective surgery (rs=0.93, P=0.002; rs=–0.99, P<0.001, respectively). In the recent implementation of performance appraisal system, the re-operation rate of day surgery was less than 0.1%, the post-discharge readmission rate of day surgery was 0%, and the satisfaction rate of day surgery patients was more than 95%, which reached 97% by 2022, higher than the average level of inpatient satisfaction in the entire hospital. Taking laparoscopic cholecystectomy, cataract phacoemulsification and intraocular lens implantation, internal fixation extraction, vocal cord polypectomy, and endoscopic gastric polypectomy as an example, the per capita total hospitalization cost and average daily cost per bed excluding drug consumption decreased by 8.87% and 17.14% compared to non-daytime mode, respectively. ConclusionUnder the guidance of national hospital performance appraisal system, day surgery has entered a rapid developing stage, but it is still necessary to promote the medical quality by standardized, precise and informationized day surgery management.
